AVC-WH-58 | Ladder & Stepladder Pre-Use Checklist
This original editable Work at Heights Part 2 inspection checklist targets real fall, access, dropped-object or rescue risks described by its printed source Word details below.
WHEN TO COMPLETE THIS ORIGINAL WAH FORM
Use for short-duration access/work only where a ladder is suitable for the task and safer equipment is not reasonably required.
PRINTED FALL-SAFETY CONTROLS — FIRST GROUP
1. Correct ladder type/length/material is selected for the access/task and electrical/environmental exposure. 2. Stiles, rungs/steps, feet, spreaders/locks, ropes/pulleys and fixings are intact with no cracks, bends, severe corrosion, contamination or makeshift repairs. 3. Labels/ID and inspection status are current enough for project control; defective ladder is quarantined. 4. Ground/support is firm, level and clear; ladder is protected from vehicles, doors, pedestrians and slipping at the base/top. 5. Extension/leaning ladder is secured and positioned to provide stable access; user does not overreach or work from an unsafe top position.
REMAINING SAFETY CHECKS AND HOLD
6. Stepladder is fully opened/locked and is not used folded/leaning unless specifically designed for that configuration. 7. User maintains controlled hand/foot contact and uses a separate method for awkward/heavy loads that prevent safe climbing. 8. Wind, rain, wet/greasy surfaces or nearby live electrical exposure does not make the ladder method unsuitable. 9. Where the task requires prolonged force, two-handed heavy work, significant sideways reach or edge exposure, a safer work platform is selected. The printed status and signature section requires OK / N/A / HOLD, evidence notes and checker RELEASE / HOLD / RE-CHECK.
FALL-PROTECTION COMPETENCY AND RESCUE LIMITS
Use the current site fall-protection plan, competent-person authorisations, verified anchors and approved access/rescue methods. Completing a paper form never makes a faulty scaffold, guardrail, ladder or rescue system safe.
