AVC-F05 | Site Safety Establishment Checklist

Source-original editable foundation, excavation and substructure Word inspection checklist for site safety establishment checklist. This source-specific form gives actual technical control points, visible inspection evidence, authorised work sequencing and HOLD decisions rather than a generic construction checklist.

WHEN TO INSPECT THIS ORIGINAL FOUNDATION OR LOWER-LEVEL WORK

Use as a practical site-control check when establishing the foundation work area. It does not replace the project safety file, statutory appointments, risk assessments, permits, competent-person duties or current legal requirements.

PRINTED GROUND CONDITION, STRUCTURE AND SITE SAFETY CONTROLS

1. Site boundary / work area is controlled so workers, visitors and the public do not enter active hazards unintentionally. 2. Gate or controlled entry point is identified and emergency access is not blocked. 3. Required project / safety information boards and emergency contact information are displayed where the team can find them. 4. Visitor / client route is identified and separated from active excavation and plant movement where practicable. 5. Hazard zones, open excavations and restricted areas are visibly barricaded or otherwise controlled. 6. Excavation warning signs / barriers are positioned before trenches or deep cuts become an open site hazard. 7. Emergency assembly / muster point is identified and kept clear. 8. First-aid equipment / contact route is known and accessible. 9. Emergency vehicle access and a route to remove an injured person have been considered. 10. Worker walking routes are reasonably clear, stable and separated from reversing / turning plant where practicable. 11. Temporary crossings, bridge boards or access points over excavations are controlled and suitable for intended use. 12. Lighting / visibility is adequate for the working times and access conditions planned. 13. Site induction route is active and new workers / visitors are briefed before entering controlled work areas. 14. Required PPE for the planned work is available and site supervision enforces its use. 15. Toolbox-talk topics reflect the actual foundation work, plant movement, excavation and changing site risks. 16. Plant operators and persons directing plant understand the work zone, blind spots and pedestrian separation.

REMAINING PRINTED CONTROLS, EVIDENCE AND RELEASE

17. Daily start-up includes a walk of barriers, access routes, excavations, water, spoil and changing conditions. 18. Daily close-out leaves excavations, tools, materials and access routes in a controlled condition after work stops. 19. Toilets, hand-washing, drinking water, shelter / eating arrangements and waste control are ready for the workforce. 20. Housekeeping prevents loose material, waste, hoses or tools from creating avoidable trip / access hazards. 21. Safe access / egress to any excavation that workers must enter has been planned and remains usable. 22. Spoil, materials and plant are positioned so trench edges are not unnecessarily loaded or obstructed. 23. Water / seepage / rain effects are checked before workers enter or inspections proceed. 24. Any deep cut, unstable side, retaining interface or other higher-risk excavation has the required competent / professional control before entry or continuation. 25. Excavations and access routes are re-checked after rain, collapse, vibration, plant movement or a material change in ground condition. 26. Weather protection and wet-access controls are adjusted when dry-weather routes become unsafe or unusable. 27. Unknown service, unexpected ground, unstable excavation or conflicting instruction triggers a stop / refer action rather than improvisation. 28. Incident, near-miss, instruction and corrective-action recording route is known to the supervisor. 29. Unresolved safety actions have a responsible person and target close-out time. 30. Public / client access is stopped or rerouted when the planned safe route is no longer safe. 31. Temporary measures left overnight are checked for wind, rain, unauthorised access and loss of barricading. 32. The supervisor is satisfied that no identified uncontrolled hazard requires work to remain on hold. Record genuine OK / N/A / HOLD, actual dimensions and tests, photo references, corrective owner and checker/specialist RELEASE / HOLD / RE-CHECK before covering, casting, loading or backfill.

FOUNDING, SHORING, ENGINEERING AND MUNICIPAL LIMITS

Use current foundation engineer and geotechnical information, approved structural and waterproofing details, competent ground/excavation and temporary support controls, verified setting-out, safe buried services, testing and pour records, and appointed inspection/municipal hold points.
