WHAT BA-C04 ACTUALLY PREVENTS

At an operating airport, a forgotten cable end, bolt, rag, packaging strip or fastening can become foreign-object debris (FOD) on an aircraft stand or operational route. BA-C04 is the genuine eight-check FOD, MATERIAL AND TOOL ACCOUNTABILITY form to prove that the contractor can account for its issued tools, consumables, site waste and end-of-shift clearance. It is a workface check rather than an informal promise that the area looks tidy.

WHEN AND WHERE TO USE THE ORIGINAL

Use before, during and at handback of an airport electrical or energy workfront with aircraft operations, restricted access and jet-blast or wind exposure. Complete AIRPORT / WORK ZONE, ASSET / LOCATION, PERMIT / WORK PACKAGE and SHIFT / DATE. Perform the physical inspection with the responsible team, match tool issue/return evidence against people, and retain restrictions on a failed row until the airport custodian can assess safe handback.

ACTUAL PRINTED CHECKPOINTS

The eight ORIGINAL Y/N/N/A controls are: (1) signed issue/return tool and loose-item count for every person entering; (2) containment/reconciliation of consumables, packaging, rags, fixings, fasteners and cut-offs; (3) waste and temporary covers secured against jet blast, wind and rotor downwash; (4) no exposed wires, loose bolts, sharp edges or loose particles; (5) prevent DC arc and energisation debris; (6) grid-interface design physical supervisor check; (7) material-collection route avoids aircraft stands and baggage/security paths; (8) final sweep and independent FOD check before area handback.

Y/N/N/A EVIDENCE AND AIRPORT HANDOVER

Each numbered row asks for YES / NO / N/A and its EVIDENCE / DEFECT REFERENCE. Close out UNRESOLVED ITEMS / RESTRICTIONS, CONTRACTOR VERIFIED BY / SIGNATURE, AIRPORT RELEASE REFERENCE, EVIDENCE REF / CORRECTIVE ACTION and DATE/TIME / STATUS. A missing tool, uncontained waste or unchecked final sweep is a real stop-work issue; do not sign a release by assumption. The airport authority issues separate operational/safety approval. This contractor record does not.
