WHEN TO PRINT AND COMPLETE
For the actual unit and shift, record the correct SITE / UNIT, CONTRACTOR, WORK ORDER / REV and WORK AREA / ZONE. Tie the workfront to the current plant risk assessment, job card, technical witness points, current isolation/permit authority and adjacent running services. Confirm the named person who can order STOP and the one who can independently release the affected station equipment.

The nominated plant supervisor, permit issuer, electrical switching officer, fire custodian, lifting engineer or technical operations owner controls official authorisation for isolation, line breaking, fire impairment, personnel access, energisation, load lifting and system reinstatement. A contractor's AVC record is not that operator permit and cannot supersede outage arrangements. Mark any source-original unresolved hazard as STOP/HOLD and escalate before the next dangerous stage.
