385 / AX-TT03 — QUARRY CRITICAL FAILURES & NEAR-MISS LESSONS
WHY THIS WORKFACE TALK MATTERS
A quarry near miss should change what crews verify, not remain only a story. This original one-page AX-TT03 toolbox talk is for quarry crushing, screening, dredging and aggregate washing. Brief workers before the relevant work starts, and repeat when a hazard, permit, shift or mine instruction changes. It links production and maintenance crews to failure mechanisms that can be missed during handover.
SIX PRINTED CREW CHECKS
1) Verify screen, crusher and conveyor guards before maintenance. 2) Assess sediment settlement, discharge permits and turbidity limits. 3) Control quarry-face rockfall, bench/crest stability and stockpile drawdown. 4) Record traceability of changed wear liners, mesh, spray nozzles and belts. 5) Test start-up interlocks, water pressure, emergency stops and dust capture. 6) Have the quarry production owner witness staged restart and confirm pond and plant safeguards.
WHEN, WHERE AND WHO USES IT
The responsible presenter briefs the crew at the actual crusher/screen line, dredging site or washing workface before a task or changed-condition restart. Connect each numbered point to the equipment, mine work order, isolation boundaries, water system and local operating controls. Workers identify unsafe conditions and confirm awareness. Signing the briefing is not a substitute for an operator permit, machine isolation or authorised release.
COMPLETE AND RETAIN THE EDITABLE WORD ORIGINAL
Fill in MINE / AREA / DATE / SHIFT and PERMIT / PRESENTER. Discuss all six items and reference unresolved problems in the applicable workpack or corrective-action record. The A4 DOCX has 16 attendance entries with NAME / EMPLOYEE OR ACCESS NO. and SIGNATURE columns. Collect signatures after the talk and file it with the actual mine shift and permit records for later review.
STOP, WITHDRAW, NOTIFY MINE CONTROL
The original STOP instruction applies to an unstable face, unguarded moving machinery, overflowing pond or trapped aggregate release. Withdraw, secure the affected task and notify mine control. Restart only after the mine-authorised official releases the affected zone or system; the contractor's attendance sheet does not grant that permission.
SINGLE, COMPLETE CHECKLIST PACKAGE OR H&S FILE
This exact original Word form is R12.95. It is included in the 386-document Surface Mining Checklist Package at R2,315.95 (about R6 per record) and the complete 635-page Surface Mining Contractor H&S File. Both larger products include this identical Word original. If you already own either, do not purchase the Single again.
