383 / AX-TT01 — QUARRY CRITICAL HAZARDS & CREW ACCOUNTABILITY
PURPOSE OF THIS WORKFACE TOOLBOX TALK
Stopped crushers, feeders and stackers can retain hydraulic, gravity and water energy. AX-TT01 is an original quarry crew briefing for crushing, screening, dredging and aggregate washing. Use it before workface access, maintenance or a new shift, and repeat following changed hazards, permits or mine instructions. It forces the crew to discuss the actual exposure rather than assume the previous team left the area safe.
SIX SOURCE-DEFINED HAZARD DISCUSSIONS
1) Verify crusher, feeder and stacker isolation covers hydraulic, gravity and water energy. 2) Assess water intake, wash-plant ponds and dredge anchoring/access as applicable. 3) Check screen, conveyor and crusher guards before maintenance. 4) Assess sediment settlement, discharge permits and turbidity limits. 5) Control quarry-face rockfall zones, bench/crest stability and stockpile drawdown. 6) Trace replacements of wear liners, mesh, spray nozzles and belts.
WHEN, WHERE AND WHO MUST ACT
The named presenter briefs the affected crew beside the actual equipment, quarry face, dredging position or washing system before task entry. Relate the six printed checks to mine procedures, equipment condition, isolation and authorised work boundaries. Every attendee should know who controls the area and where unresolved hazards are escalated. A toolbox talk is not permission to energise plant.
HOW TO COMPLETE AND KEEP THE ORIGINAL
Fill in MINE / AREA / DATE / SHIFT and PERMIT / PRESENTER. Review the numbered checks using real observations and carry open corrective actions into the site workpack. The editable A4 Word form has 16 attendance rows for NAME / EMPLOYEE OR ACCESS NO. and SIGNATURE. Collect the signatures after briefing and retain the record with the mine shift and permit evidence.
STOP, WITHDRAW AND AUTHORISED RESTART
The printed STOP / WITHDRAW / NOTIFY MINE CONTROL instruction covers unstable quarry faces, unguarded moving plant, overflowing ponds and trapped aggregate release. Stop the task and notify control. Restart requires release of the actual zone or system by a mine-authorised official; a signed talk is not an isolation certificate or site permit.

This original editable DOCX is R12.95. The same form is included in the 386-document Surface Mining Checklist Package at R2,315.95 (about R6 per form) and the complete 635-page Surface Mining Contractor H&S File currently R1,899.94. The master includes all 386 records plus the contractor management framework and specialist annexures. Test the source quality with a Single; avoid buying the form again if you own the complete file.
