WHAT THIS FORM ACTUALLY RECORDS

AX-T01 is a seven-row inspection and acceptance evidence sheet for quarry crushing, screening, dredging and aggregate washing—not a toolbox talk. It records actual measured or assessed results and supporting mine-controlled documents before machinery, a water facility or a work zone can be considered for handback. That matters when verbal assurances of 'tested' do not establish which device, reading or approval was checked.

SEVEN SOURCE-DEFINED ITEMS

1) Water intake, wash ponds and dredge anchoring/access. 2) Screen, conveyor and crusher guards before maintenance. 3) Sediment settlement, discharge permits and turbidity limits. 4) Quarry-face rockfall, bench/crest stability and stockpile drawdown. 5) Traceability of replaced wear liners, mesh, spray nozzles and belts. 6) Start-up interlocks, water pressure, emergency stops and dust capture. 7) Production-owner witnessed staged restart, including pond and plant safeguards. WHEN, WHERE AND WHO

Use this form during inspection, recommissioning or controlled handback of a quarry processing line, face, wash circuit or dredging interface. The contractor supervisor records measurements and work-order evidence. The mine release authority then reviews the submitted record before authorising any handback. Failed interlocks, absent guards, unstable ground or an unapproved discharge must remain open.

CONTROLLED RELEASE, NOT A PERMIT

A completed AX-T01 supports traceable acceptance but cannot itself grant an operating permit, discharge permission or isolation release. Attach actual test evidence and authorisations, and withhold handback until relevant defects are corrected and checked.
