381 / AX-R04 — COMPONENT COMPLIANCE & REINSTATEMENT TRACEABILITY

WHY THIS REGISTER-STYLE RECORD MATTERS

AX-R04 is a seven-row evidence sheet for changed or reinstated components on quarry crushers, screens, dredging and aggregate-wash systems. Unlike a crew talk, it provides space to tie a particular fitted part, test, permit or release decision to traceable records. A newly installed belt, nozzle or liner cannot be regarded as verified merely because maintenance reports that the machine is running.

SEVEN ACTUAL SOURCE-DOCUMENT ROWS

1) Production-owner witnessed staged restart and pond/plant safeguards. 2) Start-up interlocks, water pressure, emergency stops and dust capture tests. 3) Part traceability for changed wear liners, mesh, spray nozzles and belts. 4) Quarry-face rockfall, bench/crest stability and stockpile drawdown. 5) Sediment settlement, discharge permit and turbidity restrictions. 6) Screen, conveyor and crusher guards before opening maintenance. 7) Water intake, wash-plant ponds and dredge anchoring/access as applicable.

WHERE, WHEN AND WHO

Use this record after replacement, repair, reinstatement or inspection at the affected quarry processing line, washing pond or dredge position. The contractor supervisor records part and evidence references; the mine release authority reviews open conditions before the workface or system is handed back. Where a protective device or part lot cannot be verified, the relevant item must remain open.

Fill MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. For each numbered item, complete MINING-SPECIFIC ITEM / EVIDENCE TO TRACK, DOCUMENT / RESULT / MEASURED VALUE, and DATE / AUTHORITY / STATUS. Cite actual batch or part details, work orders, readings, inspection evidence and authority decisions. The OUTSTANDING DEFECT / NCR / REINSPECTION / RETENTION FILE REF line captures incomplete reinstatement. Close with supervisor and mine release signatures and date/release reference.

THIS IS NOT AN AUTOMATIC RESTART APPROVAL

The completed sheet supports a defensible reinstatement trail; it cannot override mine isolation, environmental discharge conditions or the official restart release. Hold work where a fitted item, guard, face condition or interlock fails verification.
