WHY THIS RESULTS REGISTER MATTERS

AX-R03 is a seven-row measured-results and evidence register for quarry crushing, screening, dredging and washing—not a general site checklist. It records what was actually observed or tested, the mine-controlled document reference and who accepts the result. That matters when a contractor says 'isolated' or 'tested' but supplies no reading, equipment reference or release decision.

SEVEN ACTUAL ITEMS IN THE SOURCE

1) Isolation of hydraulic, gravity and water energy at crushers, feeders and stackers. 2) Water intake, wash ponds and dredge anchoring/access. 3) Guards at screens, conveyors and crushers before maintenance. 4) Sediment settlement, discharge permits and turbidity restrictions. 5) Quarry-face rockfall, bench/crest stability and stockpile drawdown. 6) Traceability of replaced wear liners, mesh, spray nozzles and belts. 7) Tests of start-up interlocks, water pressure, emergency stops and dust capture.

WHERE, WHEN AND WHO COMPLETES IT

Use this original at the affected crusher line, quarry face, dredging equipment or wash circuit during execution, test verification or close-out. The contractor supervisor records genuine readings and evidence from the workpack. The mine release authority reviews results before handback. An unverified isolation, unsafe face or water discharge remains open; a completed field alone does not make a control safe.

This register supports an accountable decision; it is not a mine operating permit, lockout release or discharge approval. If interlocks, guarding, rockfall controls or readings fail, retain the defect and obtain the required official release before restart.
