WHAT THIS ACTUAL RECORD IS FOR

AX-R02 is an editable seven-line quarry defects and nonconformance evidence sheet. It helps contractors follow a failed safeguard, incorrect replacement component or unsafe plant/ground condition through corrective action and verified close-out, rather than allowing a verbal 'fixed' report to become an uncontrolled restart. Use it on crushing, screening, dredging and aggregate-wash plant work.

SEVEN PRINTED CONTROL AREAS

1) Production-owner witnessed staged restart and pond/plant safeguards. 2) Start-up interlocks, water pressure, emergency stops and dust capture. 3) Part traceability for changed wear liners, mesh, spray nozzles and belts. 4) Quarry-face rockfall, bench/crest stability and stockpile drawdown. 5) Sediment settlement, discharge permit and turbidity restrictions. 6) Guards on screens, conveyors and crushers before maintenance. 7) Water intake, wash-plant ponds and dredge anchoring/access as relevant.

WHERE, WHEN AND WHO MUST COMPLETE IT

Use the sheet when a defect, near miss, NCR or failed inspection affects the particular work order or mine zone. The contractor supervisor describes and evidences the affected control and subsequent correction; the mine release authority decides whether reinspection supports safe handback. Record the state of the actual belt, screen, pond, dredge or quarry face, not generic 'compliant' language.

HOW TO DOCUMENT CORRECTIVE ACTION

Fill MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. In the numbered table, use MINING-SPECIFIC ITEM / EVIDENCE TO TRACK, DOCUMENT / RESULT / MEASURED VALUE and DATE / AUTHORITY / STATUS to reference the original failed check, corrected result, test or photo. The separate OUTSTANDING DEFECT / NCR / REINSPECTION / RETENTION FILE REF line links the formal defect and evidence record. Obtain contractor-supervisor and mine-release-authority signatures with date/release reference only after the required review.

CORRECTION IS NOT AUTOMATIC RELEASE

An NCR cannot be considered closed merely because a guard was refitted or a pond pumped down. The evidence and mine-controlled approval must support safe release. This form does not itself issue an isolation certificate, environmental discharge permit or operating authorisation.
