378 / AX-R01 — COMPETENCE, PERMITS & PLANT IDENTIFICATION

THE PURPOSE OF THIS SOURCE RECORD

AX-R01 is a seven-row quarry evidence sheet linking the specific plant, work zone, contractor supervisor, permit and supporting documents to safety checks. It covers crushing, screening, dredging and washing operations. A test or isolation record for the wrong conveyor, quarry face or work order cannot establish that the actual task is ready. This is an identification and authorisation trace, not a generic attendance register.

SEVEN ORIGINAL PLANT-CONTROL ROWS

1) Isolation of crusher, feeder and stacker hydraulic, gravity and water energy. 2) Water intake, wash ponds and dredge anchoring/access. 3) Screen, conveyor and crusher guards before maintenance. 4) Sediment settlement, discharge permits and turbidity restrictions. 5) Quarry-face rockfall, bench/crest stability and stockpile drawdown. 6) Traceability of wear liners, mesh, spray nozzles and belts. 7) Start-up interlocks, water pressure, emergency stops and dust capture tests.

WHERE, WHEN AND WHO

Use the form when mobilising or verifying the particular quarry work order, before controlled workface access, maintenance or handback. The contractor supervisor records the correct equipment and source documents and points to any needed competence or mine permit evidence. The mine release authority checks the evidence before issuing release. Missing permits, unidentified plant or incomplete controls must remain open.

HOW TO COMPLETE THE ACTUAL WORD DOCUMENT

Enter MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. AX-R01 does not itself certify someone competent, create an operating permit or release isolated machinery. Refer to actual approvals and retain the record against the exact mine zone. A failed safeguard or unapproved discharge must not be marked accepted.
