A FIVE-GATE HOLD/RELEASE RECORD

AX-H02 is a mine-controlled independent inspection and witness form, not a routine toolbox talk. Contractors record evidence before the quarry crusher, screen, dredging position or washing plant is handed back. The contractor verifies its work, but only the designated mine authority issues plant/area release. The distinction matters after guards were opened, equipment replaced or environmental and ground safeguards changed.

FIVE SOURCE-DEFINED HOLD TRIGGERS

1) Verify screen, conveyor and crusher guards before maintenance access. 2) Check sediment settlement, discharge permits and turbidity limits. 3) Control quarry-face rockfall, bench/crest stability and stockpile drawdown. 4) Trace replacements of wear liners, mesh, spray nozzles and belts. 5) Test start-up interlocks, water pressure, emergency stops and dust capture. These are separate evidence gates; a missing test or inspection must not be marked released.

WHERE, WHEN AND WHO MUST REVIEW

Use the form before final handback or recommissioning of the affected quarry work order or mine zone. The contractor supervisor assembles test evidence, inspection reports, part references and permits. A witness records what was verified. The designated mine release authority then reviews all gates and any defects before authorising handback. Signing as a contractor cannot independently release the system.

HOW TO COMPLETE THE ORIGINAL WORD FORM

Enter MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. For each of five rows, complete HOLD TRIGGER / EVIDENCE GATE, REQUIRED EVIDENCE / VERIFICATION, and RESULT / REFERENCE / WITNESS. Record actual document IDs, observed results and witness details. The RELEASE instruction requires all gates verified and defects resolved. Close with contractor-supervisor and mine-release-authority signatures plus date/release reference.

WHEN WORK MUST REMAIN ON HOLD

Do not bypass a failed interlock, absent guard, unsafe face or unapproved discharge because production needs a restart. AX-H02 supports the control decision but is not an isolation cancellation, environmental permit or automatic mine approval. Retain the supporting records.
