A FORMAL FIVE-GATE MINE WORK HOLD

AX-F01 controls the boundary between contractor verification and mine-authorised release on quarry crushing, screening, dredging and aggregate-washing work. It is a five-gate hold/release record, not a toolbox talk or automatic permit. Work involving stored energy, moving plant, quarry faces or dredge/water systems must remain held where the required technical evidence or mine permission has not been supplied.

FIVE ACTUAL HOLD TRIGGERS

1) Confirm crusher, feeder and stacker isolation covers hydraulic, gravity and water energy. 2) Assess water intake, wash-plant ponds and dredge anchoring/access where applicable. 3) Verify screen, conveyor and crusher guard locations before maintenance begins. 4) Assess sediment settlement, discharge permits and turbidity restrictions. 5) Control quarry-face rockfall, bench and crest stability and stockpile drawdown. Each trigger has space for required proof and a separate result, witness or reference.

WHEN AND WHERE TO USE AX-F01

Use this form at the specified mine zone and work order before technical handback, restart, access to a previously held area or close-out of work that required mine release. The contractor supervisor first checks the evidence against the actual crusher, feeder, quarry bench, wash pond or dredge position. The authorised mine official—not the contractor—must issue the final release. Unresolved controls mean the hold remains.

FILLING THE ORIGINAL A4 WORD HOLD SHEET

Enter MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. For each of the five printed gates, complete HOLD TRIGGER / EVIDENCE GATE, REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. Record source permits, isolation records, test findings and the person witnessing the check. The RELEASE instruction requires every gate verified and defects resolved. Contractor supervisor and mine release authority sign separately, with date and release reference.

WHY A SIGNATURE ALONE IS INSUFFICIENT

Do not treat a completed sheet as authority to cancel an isolation or ignore a defective guard, unsafe face or restricted water discharge. It records the evidence leading to the mine official's decision. Only a valid mine-controlled release permits the affected plant or area to return to service.
