375 / AX-C03 — QUARRY EMERGENCY SAFEGUARDS & WITHDRAWAL READINESS
WHY THIS ORIGINAL FORM MATTERS
AX-C03 is a seven-point Y / N / N/A emergency-readiness checklist for quarry crushing, screening, dredging and aggregate washing. Before crews enter or restart a workface, it tests whether missing guards, defective stops, unstable faces, wash-pond conditions or uncertain mine release could make withdrawal unsafe. It records actual controls and exceptions, rather than promising that a signed form alone makes an emergency safe.
WHAT THE ACTUAL CHECKLIST VERIFIES
The seven printed checks cover: 1) production-owner witnessed staged restart and pond/plant safeguards; 2) tested interlocks, water pressure, emergency stops and dust capture; 3) traceable wear-liner, mesh, spray-nozzle and belt replacements; 4) controlled rockfall, bench/crest stability and stockpile drawdown; 5) settlement, discharge permits and turbidity restrictions; 6) screen, conveyor and crusher guards before maintenance; 7) water intake, wash ponds and dredge anchoring/access where applicable. Each item needs workface evidence, not an assumed pass.
WHERE, WHEN AND WHO
Use before a controlled restart, exposure to processing machinery or changed quarry instructions at the exact work order and mine zone. The contractor supervisor checks the processing line, ground and water interface and puts unresolved hazards before mine control. The mine release authority determines whether the affected system may return to use.
HOW TO COMPLETE THE ORIGINAL WORD RECORD
Enter MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Each numbered WORKFACE VERIFICATION / REQUIRED CONTROL row has Y, N, N/A and EVIDENCE / DEFECT / ACTION. Mark the actual result, explain every NO and N/A, cite the current mine work order/COP and attach relevant proof. Sign-off separates the contractor supervisor from the mine release authority, including date and release reference.
HOLD POINTS AND AUTHORITY LIMITS
The printed HOLD rule requires stopping for missing clearance, isolation, protection or critical evidence and reassessing changed conditions. A failed emergency stop, unstable face or inaccessible pond route cannot be cleared by an unexplained tick. This checklist is not a mine permit, formal rescue plan or independent restart authorisation.
SINGLE, CHECKLIST PACKAGE OR FULL SAFETY FILE
The editable original Word form costs R12.95 and is included in the 386-document Surface Mining Checklist Package and the complete Surface Mining Contractor H&S File. The complete file adds the contractor management framework and specialist annexures. Try the Single to see the actual form; owners of the complete collection already have it.
