374 / AX-C02 — QUARRY CHANGED CONDITIONS, WEATHER & EXPOSURE

WHEN THIS CHECKLIST IS NEEDED

Rain, changing pond levels, ground movement or plant intervention can invalidate a previous assessment. AX-C02 is a seven-point Y / N / N/A checklist for rechecking quarry crushing, screening, dredging and aggregate washing. Its title includes weather and exposure, but its actual rows assess plant, water, ground, stored-energy and restart safeguards—not weather measurements.

SEVEN CONTROLS PRINTED ON THE SOURCE FORM

1) Screen, conveyor and crusher guards before maintenance. 2) Sediment settlement, discharge permits and turbidity limits. 3) Quarry-face rockfall zones, bench/crest stability and stockpile drawdown. 4) Traceable changed wear liners, mesh, spray nozzles and belts. 5) Tests of start-up interlocks, water pressure, emergency stops and dust capture. 6) Quarry production-owner witnessed staged restart and pond/plant safeguards. 7) Isolation of hydraulic, gravity and water energy in crushers, feeders and stackers. Compare every item with today's changed conditions.

WHERE, WHEN AND BY WHOM

The contractor supervisor uses AX-C02 before work resumes after a changed workface, water condition, shift instruction or maintenance boundary. Inspect the actual crusher, face, dredge or wash circuit rather than trusting an old shift record. The mine release authority reviews any open clearance or exposure issues before recommencement.

COMPLETING THE ORIGINAL EDITABLE WORD FILE

Enter MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Each numbered WORKFACE VERIFICATION / REQUIRED CONTROL row has Y, N, N/A and EVIDENCE / DEFECT / ACTION. Explain each NO or N/A against the current mine work order/COP; attach inspection, permit or test evidence. The original close-out requires both contractor-supervisor and mine-release-authority signatures, date and release reference.

HOLD AND RELEASE RESPONSIBILITIES

The printed HOLD notice applies when clearance, isolation, protection or critical evidence is missing and requires changed conditions to be reassessed. Do not resume unguarded plant, enter an unstable face or ignore discharge limits because an earlier record passed. This is not an operating permit or automatic mine restart release.

THIS SINGLE OR THE COMPLETE MINING SYSTEM

This original editable A4 DOCX is R12.95 and already included in the 386-document Surface Mining Checklist Package and the complete Surface Mining Contractor H&S File. The full file adds the contractor management core and specialist annexures. Try a Single to inspect the original; don't repurchase an included document.
