373 / AX-C01 — QUARRY PRE-START & MINE INTERFACE CHECKLIST
SAFE QUARRY WORKFACE ENTRY
AX-C01 is the original seven-point Y / N / N/A pre-start and mine-interface checklist for quarry crushing, screening, dredging and washing. Before contractors approach production-owned equipment or mine-controlled ground, it verifies the specific isolation, guards, water and face controls. It is a site-readiness record, not an operating permit or a record of subsequent acceptance tests.
SEVEN PRINTED PRE-START VERIFICATIONS
1) Crusher, feeder and stacker isolation of hydraulic, gravity and water energy. 2) Water intake, wash-plant ponds and dredge anchoring/access. 3) Screen, conveyor and crusher guards before opening maintenance. 4) Sediment settlement, discharge permits and turbidity restrictions. 5) Quarry-face rockfall, bench/crest stability and stockpile drawdown. 6) Traceable changes to wear liners, mesh, spray nozzles and belts. 7) Tests of start-up interlocks, water pressure, emergency stops and dust capture. Each item is a workface check, not a generic box.
WHERE, WHEN AND WITH WHOM
The contractor supervisor completes this form at the exact mine work order and zone before first access, machine maintenance or a changed permit. Inspect the actual crusher, wash circuit, dredge position or quarry face with the people exposed. Liaise with the mine representative responsible for isolation and release; the previous shift's permission cannot be assumed to carry over.
HOW TO COMPLETE THE EDITABLE WORD SOURCE
Fill MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. For each WORKFACE VERIFICATION / REQUIRED CONTROL row select Y, N or N/A and enter EVIDENCE / DEFECT / ACTION. Explain all NO/N/A results against the current mine work order/COP, attaching permits or test evidence. The original close-out has separate contractor-supervisor and mine-release-authority signatures, date and release reference.
A FAILED PRE-START REMAINS ON HOLD
AX-C01 directs HOLD when clearance, isolation, physical protection or essential evidence is absent, and calls for changed conditions to be reassessed. Do not commence unguarded maintenance, enter an unstable face or energise machinery because seven boxes have been ticked. The authorised mine release remains decisive.
SINGLE, PACKAGE OR COMPLETE SAFETY FILE
