START WITH THE CORRECT MINE WORK ORDER

AX-00 links a specific quarry mine zone, contractor supervisor, permit and responsible owner to the work being activated. This six-question source form is for opening the quarry workpack before crushing, screening, dredging, washing or aggregate cleaning begins. It is an activation and acceptance-evidence record, not a toolbox talk or standalone permission to enter production-owned machinery.

SIX CONTROL QUESTIONS PRINTED IN THE FORM

1) Confirm crusher, feeder and stacker hydraulic, gravity and water-energy isolation. 2) Assess water intake, wash-plant ponds and dredge anchoring/access. 3) Verify screen, conveyor and crusher guards before maintenance. 4) Assess sediment settlement, discharge-permit and turbidity restrictions. 5) Control quarry-face rockfall, bench/crest stability and stockpile drawdown. 6) Trace changed wear liners, mesh, spray nozzles and belts. WHERE, WHEN AND WHO ACCEPTS IT

Use AX-00 at mobilisation of a particular quarry task before the crew enters the mine-controlled zone. The contractor supervisor identifies the plant, isolation interface and actual scope; the mine owner or authorised representative confirms the relevant work controls. The same form allows later reviewers to trace what was accepted before work started, rather than relying on a vague verbal instruction.

Enter MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. AN ACTIVATED WORK ORDER IS NOT A RELEASE

The record establishes what permission and evidence must exist, but does not replace the mine's isolation, discharge or area-release procedures. A blank control reference means a required assurance is unproven. Retain the signed activation alongside the permit/workpack before handing workers access to moving machinery or unstable ground.
