This original one-page editable Word register records actual results and assessed evidence for surface-mine rail loadout, stockyard and bulk-material dispatch. It identifies the mine-controlled documents, measured values, responsible authority and status for each critical workface control.
WHY A MEASURED EVIDENCE REGISTER MATTERS
A verbal 'track safe', 'chute isolated' or 'weighbridge calibrated' is no substitute for verified results. Moving trains, live traction, energised stackers and unstable stockpiles can expose contractors where operating release is assumed. AW-R03 records the checks and witnesses that support a mine-authorised decision.

1. Rail infrastructure authority releases track occupation and prohibits movements. 2. Shunting, stockyard traffic and conveyor routes are segregated from work crews. 3. Wagon-loading chute, stacker/reclaimer and tripper motions are positively isolated. 4. Overhead traction, crossings and traction-return electrical hazards are identified.
5. Stockpile stability, engulfment and suspended-reclaimer risks are controlled. 6. Track gauge, switches, signalling and weighbridge calibration evidence is recorded. 7. Interlocking and train-movement tests after modifications are witnessed. These are the seven original numbered subjects, not a generic rail checklist.

Complete separate CONTRACTOR SUPERVISOR / SIGNATURE and MINE RELEASE AUTHORITY / SIGNATURE fields with DATE / RELEASE REF. Missing tests or unverified defects require escalation; a contractor's results register cannot authorise train movements.
