AW-H02 is a five-gate independent-witness hold-and-release record for underground industrial cleaning and waste. It is different from an inspection register: applicable controls require evidence and an identified witness before the host mine issues its own release.
WHEN AND WHERE THE MINE CONTRACTOR USES THIS
Use it for an underground cleaning, wash-down, jetting or waste-work hold point before equipment or an area is handed back. Identify MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. The printed source expressly records operator release by reference only.
THE FIVE ORIGINAL HOLD GATES
1. No use of solvents, polishers or water methods outside approved material procedure
2. Cleaning does not damage support tags, sensors, cable protection or fire barriers
3. Wash water and sludge managed to avoid dewatering or water-quality impairment

HOW TO COMPLETE THE ACTUAL SOURCE TABLE
The printed table reads HOLD TRIGGER / EVIDENCE GATE, REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. Record real proof and the actual witness for every applicable hold; a failed or absent result remains an unresolved gate.
WHAT CAN GO WRONG AT THE ACTUAL WORKFACE
A completed cleaning shift is not a clearance if a pressure jet remains unisolated or washwater could impair dewatering. The source's five gates require actual method approval, protected services, wastewater control, surface waste proof and a witnessed jetting check.
DEFECTS, WITNESSES AND MINE RELEASE AUTHORITY
The printed RELEASE CONDITION requires all applicable hold items PASS, evidence traceable to the approved mine work order, corrected defects witnessed, and release by the named mine authority.
