362 / AW-H02 — RAIL INDEPENDENT INSPECTION, WITNESS & HANDBACK
AW-H02 is an original one-page Word hold-and-release form for independent inspection, witness evidence and controlled handback at mine rail loadout, stockyards, sidings, stackers and reclaimers. It separates the contractor's verification from the designated mine authority's approval to release a plant area or system.
WHY AN INDEPENDENT RAIL HANDOVER GATE IS NEEDED
A train path or loading system may appear repaired while isolation, live traction, reclaimer stability or interlock tests remain unverified. AW-H02 records specific evidence gates and witness references before the rail/stockyard owner resumes operation. This is NOT a crew toolbox talk, routine tick checklist or replacement for host railway permission.
FIVE ACTUAL HOLD TRIGGERS IN THE SOURCE
1. Positive isolation of wagon-loading chutes, stacker/reclaimer and tripper motions. 2. Identification of overhead traction, crossings and traction-return electrical hazards. 3. Control of stockpile stability, engulfment and suspended reclaimers before workface entry. 4. Evidence of track gauge, switch, signalling and weighbridge calibration. 5. Witnessed interlocking and train movement tests after modifications.
WHAT THE ORIGINAL FORM REALLY CONTAINS
Identify MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Its printed columns are HOLD TRIGGER / EVIDENCE GATE, REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. For each of the five gates write the actual supporting isolation, test, measurement, permit and witness references, attaching documents where appropriate.
The printed RELEASE condition requires all gates verified and defects resolved before an authorised mine official signs a valid work release. Separate CONTRACTOR SUPERVISOR / SIGNATURE and MINE RELEASE AUTHORITY / SIGNATURE fields plus DATE / RELEASE REF establish the release record. The contractor can verify evidence but cannot issue mine/rail operating permission. Unresolved critical evidence means HOLD.
