AW-F01 — MINE CLEANING APPROVED WORK PACKAGE & HOLD/RELEASE RECORD
AW-F01 is the approved-work-package and hold/release sheet for underground industrial cleaning, dust and waste services. Its five real hold gates need mine work-order evidence and witnessed verification before an area, airway or affected service can be released.
WHERE AND WHEN THE SOURCE FORM IS USED
Use it where underground cleaning or waste operations involve a mine-owned hold at inspection or handback. Fill in MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR, connecting each gate with the actual operator-controlled work order.
FIVE ORIGINAL WITNESS HOLD POINTS
1. Cleaning area and adjacent airways released by mine ventilation and area authority
2. Dust, silica, diesel particulate and chemical exposure control defined
3. No use of solvents, polishers or water methods outside approved material procedure
4. Cleaning does not damage support tags, sensors, cable protection or fire barriers
5. Wash water and sludge managed to avoid dewatering or water-quality impairment
HOW TO COMPLETE THE EXACT TABLE
The original HOLD TRIGGER / EVIDENCE GATE, REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS columns demand actual proof and a named witness for five holds. Operator-owned work release is recorded by reference only, never granted by this contractor form.
SPECIFIC MINE-WORKFACE RISK
Ventilation/adjacent-airway release and defined silica/diesel/chemical controls are separate hold items. Only approved cleaning methods may be used, and work must preserve support tags, monitoring, cable/fire barriers and dewatering. Never self-release a failed or unwitnessed gate.
DEFECTS, STOP RULES AND MINE RELEASE
RELEASE CONDITION requires all applicable holds PASS, evidence traceable to the approved mine work order, corrected defects witnessed and valid release issued by the named mine authority. Contractor supervisor and host mine sign where required, with Date/release reference.
