AW-C03 — MINE CLEANING CONDITION CHANGE & EMERGENCY CHECKLIST
AW-C03 is the source-original change-of-condition and emergency-protection checklist for underground industrial cleaning. It requires seven new workface checks after a changed hazard, permit, mine instruction or shift instead of carrying forward yesterday's cleaning clearance.
WHERE AND WHEN THE SOURCE FORM IS USED
Use AW-C03 at the mine workface, level, jetting route or waste zone when conditions change. Enter MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR against the new shift and the mine-approved COP/SOP.
SEVEN ACTUAL UNDERGROUND CHECKS
1. Mine area owner accepts clean condition, remaining waste and restored equipment
2. High-pressure jetting requires dedicated line isolation, hose integrity and exclusion zones
3. Waste categories, containers and transfer records traceable to surface disposal
4. Wash water and sludge managed to avoid dewatering or water-quality impairment
5. Cleaning does not damage support tags, sensors, cable protection or fire barriers
6. No use of solvents, polishers or water methods outside approved material procedure
7. Dust, silica, diesel particulate and chemical exposure control defined
HOW TO COMPLETE THE EXACT TABLE
The original WORKFACE VERIFICATION / REQUIRED CONTROL table has Y, N and N/A response columns and EVIDENCE / DEFECT / ACTION. Assess each source item against current mine SOP/COP and approved work order, explain NO or N/A decisions and attach evidence. This checklist is not a work permit.
SPECIFIC MINE-WORKFACE RISK
The area owner's prior acceptance may cease to apply after a change in jetting exclusion, washwater route, ground support or monitoring. Check remaining waste, pressure hoses, surface manifests, solvents and dust/diesel exposure. Missing emergency clearance or other critical proof requires HOLD.
DEFECTS, STOP RULES AND MINE RELEASE
The printed HOLD instruction requires stopping where mine clearance, isolation, emergency systems, support condition or critical evidence is absent, and reassessment after a changed shift or conditions. Use REVIEW / CLOSE-OUT AND EVIDENCE REFERENCES and accountable supervisor/mine signatures where required.
