AW-00 — MINE CLEANING ANNEXURE ACTIVATION & EVIDENCE INDEX
AW-00 is the original annexure activation and evidence index for underground industrial cleaning, dust and waste services. It establishes whether the trade annexure is applicable to the awarded contract and approved mine zone and identifies evidence owners without inventing new permit obligations.
WHERE AND WHEN THE SOURCE FORM IS USED
Use AW-00 while setting up the contractor's actual awarded scope and safety file. Record MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR, then identify the applicable acceptance evidence for the six original requirements.
SIX ORIGINAL ACTIVATION REQUIREMENTS
1. Cleaning area and adjacent airways released by mine ventilation and area authority
2. Dust, silica, diesel particulate and chemical exposure control defined
3. No use of solvents, polishers or water methods outside approved material procedure
4. Cleaning does not damage support tags, sensors, cable protection or fire barriers
5. Wash water and sludge managed to avoid dewatering or water-quality impairment
6. Waste categories, containers and transfer records traceable to surface disposal
HOW TO COMPLETE THE EXACT TABLE
Complete the original ACTIVATION REQUIREMENT, EVIDENCE / ACCEPTANCE REFERENCE and STATUS / OWNER table for each of the six relevant controls. Link each activation condition to the site document, acceptance authority and outstanding owner.
SPECIFIC MINE-WORKFACE RISK
If ventilation release, dust/chemical exposure control, approved solvents, protection of mine sensors/fire systems, water handling or surface waste custody is absent, record the actual evidence and accountable owner. An activated annexure does not replace a valid mine access or work permit.
DEFECTS, STOP RULES AND MINE RELEASE
The source ends with REVIEW / CLOSE-OUT AND EVIDENCE REFERENCES, contractor and host mine authority signature spaces where required and Date / release reference. The index alone never authorises underground entry or service release.
