A new shaft shift may inherit rope tension, an incomplete brake test or an unsafe lifting exclusion. AV-TT03 is the editable shift-change and near-miss briefing that turns the previous crew's lessons into a specific isolation, inspection and reinstatement discussion.
WHERE AND WHEN THE MINE CREW USES THIS
Present it to the incoming mine winder maintenance and rigging crew before entering the controlled shaft work area. Identify changed instructions, unresolved incidents, continuing isolations and actions left from the prior shift. Repeat following changed mine permits, hazards or shifts.
SIX ACTUAL WORKFACE BRIEFING CONTROLS
1. Energy and stored tension in ropes, brakes and counterweights isolated
2. Independent anchorage, lifting, dropped-object and shaft barricade plans approved
3. Inspection scope covers guides, headgear, ropes, stations and signalling as applicable
4. Mine authorised competent inspector witnesses safety-device and brake tests

IMPORTANT SHAFT-ENGINEERING DECISION
Check independent lifting anchors, dropped-object barriers, inspection scope, competent mine-witnessed brake testing and escape separation before shift turnover is accepted. A previous worker's verbal assurance never substitutes for mine-owned equipment clearance.
STOP, WITHDRAW AND OPERATOR RELEASE
The source lists unknown conveyance status, unsecured shaft opening, unproved brake or absent winder control as STOP / WITHDRAW / NOTIFY MINE CONTROL triggers. The mine-authorised official must release the affected zone or system before restart.
