AV-R03 — SHAFT & WINDER WORK RESULTS, MEASUREMENTS AND EVIDENCE REGISTER
AV-R03 measures and records the actual results of shaft and winder maintenance. Its seven printed points start with mine-issued work possession and conveyance rescue restrictions, then check energy isolation, lifting protection and equipment inspection before a witnessed safety-device test.

ACTUAL SHAFT AND WINDER FAILURE CONTROL
A completed rope repair does not prove a brake's witnessed acceptance test. Separately record shaft possession, safe evacuation, lifting barricades, inspection results and competent mine witness; missing readings or certificates remain defects.
