PURPOSE OF THIS EXACT AIRPORT NDT ORIGINAL
AV-H02 is the source-original EIGHT-GATE CRITICAL HOLD-POINT AND INDEPENDENT RELEASE RECORD for inspection, NDT and commissioning near live airport systems. A contractor's completed testing certificate cannot alone remove a radiographic exclusion, release pressure/electrical energy or place a system back into airport operations. This form separates the actual acceptance evidence, competent independent witness and the airport-authorised return-to-service decision.
WHEN / WHERE THIS FORM IS REQUIRED
Use at a designated STOP/HOLD in the inspection and test plan before returning an affected asset, plant or service to operational use. Fill AIRPORT/EQUIPMENT, WORK PACKAGE/ITP, HOLD TRIGGER/TIME and AIRPORT PERMIT REF. Confirm design revision and system boundary, the relevant radiographic exclusion, pressure or electrical state, approved specialist equipment and current operating restrictions with the independent reviewer.
