AV-H02 — SHAFT & WINDER INDEPENDENT WITNESS HOLD AND RELEASE
A contractor's signature cannot reopen an underground shaft or return a winder to service. AV-H02 is the original editable five-gate independent-witness hold-and-release form for stored energy, lifting, technical inspection, competent brake tests and protected worker access.
WHEN AND WHERE THIS RECORD IS USED
Use it at a mine-required inspection hold or plant-release boundary during winder, rope or shaft engineering. Enter MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. The source says operator-owned work release is recorded by reference only.
FIVE INDEPENDENT WITNESS HOLD GATES
1. Energy and stored tension in ropes, brakes and counterweights isolated
2. Independent anchorage, lifting, dropped-object and shaft barricade plans approved
3. Inspection scope covers guides, headgear, ropes, stations and signalling as applicable

HOW THE ORIGINAL TABLE IS COMPLETED
Complete the printed HOLD TRIGGER / EVIDENCE GATE, REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS columns. Each of five gates needs the relevant actual document and named witness, with a failed gate remaining on HOLD.
ACTUAL SHAFT AND WINDER FAILURE CONTROL
The original RELEASE CONDITION requires all applicable holds PASS, evidence traceable to the approved work order and witnessed correction of defects before the named mine authority issues valid release. Contractor/host signatures and date/release reference document, not create, that authority.
DEFECTS, SIGNATURES AND MINE RELEASE
The operator owns plant and area release; this form verifies evidence and actual witness. Review/close-out references and contractor and host mine signatures where required must match the official Date / release reference.
