AV-F01 — SHAFT & WINDER APPROVED WORK PACKAGE AND HOLD/RELEASE RECORD
AV-F01 is the original approved-work-package and hold/release record for underground shaft infrastructure, conveyance maintenance and winder engineering. It requires five actual mine-owned evidence gates before a contractor's work can be handed back.
WHERE AND WHEN THIS SHAFT DOCUMENT IS USED
Use this before releasing an approved shaft work order after rope, brake, guide, headgear or station engineering. Enter MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR, matching each hold gate to that mine-approved workpack.
FIVE PRINTED WORK-PACKAGE HOLD GATES
1. Winder and shaft work possession issued by mine shaft authority
2. Personnel conveyance restrictions, emergency movement and rescue plan communicated
3. Energy and stored tension in ropes, brakes and counterweights isolated
4. Independent anchorage, lifting, dropped-object and shaft barricade plans approved
5. Inspection scope covers guides, headgear, ropes, stations and signalling as applicable
HOW TO COMPLETE THE ACTUAL SOURCE TABLE
The original table has HOLD TRIGGER / EVIDENCE GATE, REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. Record a genuine mine document, observed result and named witness for every applicable gate. The contractor cannot issue the operator-owned work release.
ACTUAL MINE SHAFT RISK AND RECORD
The five holds start with work possession and safe personnel conveyance/rescue arrangements. Prove rope/brake/counterweight isolation, independent anchorage and dropped-object barricades, and the specific guide/rope/headgear/signalling inspection scope. Missing evidence leaves the work on HOLD.
HOLD, CLOSE-OUT AND MINE AUTHORITY
RELEASE CONDITION: all applicable holds PASS, evidence traces to the approved mine work order, defects are corrected and witnessed and the named mine authority issues valid release. Contractor/host mine sign-offs and Date / release reference document the decision.
