AV-C02 — SHAFT & WINDER SHIFT AND AREA READINESS CHECKLIST
AV-C02 verifies whether a shaft and winder engineering area is ready for the current shift. Its seven source checks begin with conveyance restrictions and rescue arrangements and finish with mine shaft-authority reinstatement, making it different from an ordinary equipment inspection.
WHERE AND WHEN THIS SHAFT DOCUMENT IS USED
Complete the checklist before a new shaft maintenance, rigging or winder shift and repeat it after a changed permit or hazard. Enter MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR, identifying the real station and current mine instructions.
SEVEN PRINTED SHAFT WORKFACE CHECKS
1. Personnel conveyance restrictions, emergency movement and rescue plan communicated
2. Energy and stored tension in ropes, brakes and counterweights isolated
3. Independent anchorage, lifting, dropped-object and shaft barricade plans approved
4. Inspection scope covers guides, headgear, ropes, stations and signalling as applicable
5. Mine authorised competent inspector witnesses safety-device and brake tests
6. Worker access and evacuation remain separate from conveyance work envelope

HOW TO COMPLETE THE ACTUAL SOURCE TABLE
Use Y, N and N/A under the original WORKFACE VERIFICATION / REQUIRED CONTROL table and enter the actual EVIDENCE / DEFECT / ACTION for each of seven rows. Confirm the mine's current COP/SOP and approved work order; an unchecked restriction is not a safe assumption.
ACTUAL MINE SHAFT RISK AND RECORD
Personnel movement must be controlled before rope/brake tension isolation, lifting barriers, inspection scope and competent witnessed safety-device tests. Separated evacuation and a signed reinstatement record remain distinct requirements. STOP/HOLD where operator clearances or critical proof are absent.
HOLD, CLOSE-OUT AND MINE AUTHORITY
The printed HOLD rule says STOP where mine-authorised clearance, isolation, emergency systems, support conditions or critical proof are absent. Reassess after a changed shift or conditions. Record actual review evidence and supervisor/mine sign-offs where required.
