AV-C01 — SHAFT & WINDER WORKFACE PRESTART CHECKLIST
AV-C01 is the original workface prestart checklist for underground shaft and winder engineering. Before a fitter or rigger enters the zone, its first check requires mine shaft-authority work possession; six further checks address movement, energy, lifting, inspection and safe access.
WHERE AND WHEN THIS SHAFT DOCUMENT IS USED
Use the editable source before starting work on ropes, counterweights, brake systems, shaft stations, headgear or guides. Record MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR, including the current mine work possession reference.
SEVEN PRINTED SHAFT WORKFACE CHECKS
1. Winder and shaft work possession issued by mine shaft authority
2. Personnel conveyance restrictions, emergency movement and rescue plan communicated
3. Energy and stored tension in ropes, brakes and counterweights isolated
4. Independent anchorage, lifting, dropped-object and shaft barricade plans approved
5. Inspection scope covers guides, headgear, ropes, stations and signalling as applicable
6. Mine authorised competent inspector witnesses safety-device and brake tests
7. Worker access and evacuation remain separate from conveyance work envelope
HOW TO COMPLETE THE ACTUAL SOURCE TABLE
For each of the seven original WORKFACE VERIFICATION / REQUIRED CONTROL items, tick Y, N or N/A, and enter EVIDENCE / DEFECT / ACTION. Explain deficiencies and not-applicable claims against the real mine SOP/COP and approved work order. The record does not grant work permission.
ACTUAL MINE SHAFT RISK AND RECORD
A vacant station does not mean the conveyance is stopped or the ropes safely isolated. The source also requires approved lift barricades, inspection of guides/headgear/ropes/signalling, mine competent brake-test witnesses and separation of workers from the conveyance. Missing clearance triggers HOLD.
HOLD, CLOSE-OUT AND MINE AUTHORITY
The printed HOLD rule says STOP where mine-authorised clearance, isolation, emergency systems, support conditions or critical proof are absent. Reassess after a changed shift or conditions. Record actual review evidence and supervisor/mine sign-offs where required.
