It helps decide whether this specialist annexure applies to the actual awarded contract and approved mine zone, with six responsible evidence lines.
WHERE AND WHEN THIS SHAFT DOCUMENT IS USED
Use the form while preparing the contractor's mine safety file and workpack before undertaking shaft/winder tasks. Record MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR so the annexure matches the scope and named mine authority.

HOW TO COMPLETE THE ACTUAL SOURCE TABLE
The original three-column index is ACTIVATION REQUIREMENT, EVIDENCE / ACCEPTANCE REFERENCE and STATUS / OWNER. Enter actual mine-approved work possession, movement restrictions, isolation, lifting plans, inspection scope and brake-test evidence, identifying who owns each requirement.
ACTUAL MINE SHAFT RISK AND RECORD
Activating a trade annexure does not grant entry or winder control. If shaft possession, rescue arrangements, counterweight isolation or an approved lift plan is missing, record the gap and obtain mine-authorised action. Contractor and host mine signatures remain subject to actual mine permission.
HOLD, CLOSE-OUT AND MINE AUTHORITY
Review/close-out references and contractor supervisor and host mine authority signatures are included where required, with Date / release reference. The evidence index itself never grants mine access or service release.
