AU-TT03 — SHOTCRETE, LINING & GROUTING SHIFT CHANGE AND NEAR MISS TALK
An incoming shotcrete crew can inherit pressurised hoses, unfinished curing holds or changed ventilation. AU-TT03 is the editable original shift-change and near-miss toolbox talk, connecting six workface controls to engineer-signed acceptance.
WHERE AND WHEN THIS MINE FORM IS USED
Present it before an incoming rehabilitation, spraying or grout-injection shift resumes at a mine heading. Review previous rebound, substrate condition, water routes and outstanding tests, repeating the briefing after any changed permit or hazard.
SIX PRINTED TOOLBOX CONTROLS
1. High-pressure hose, rebound and splash exposures controlled
2. Ventilation and dust/mist capture adequate for spray and curing operations
3. No workers exposed to unsupported ground during substrate preparation
4. Substrate tests, thickness checks, strength verification and cure holds defined
5. Runoff and washout protected against mine drainage and contamination
6. Mine engineer signs thickness, strength and zone re-entry release
HOW TO COMPLETE THE SOURCE RECORD
Enter MINE / AREA / DATE / SHIFT and PERMIT / PRESENTER, discuss all six controls in their exact printed order and collect up to 16 worker names/access numbers and signatures. Log real near-miss corrective actions separately.
MINE-WORKFACE HAZARD AND QUALITY DECISION
The source starts with hose/rebound containment, then dust/mist ventilation, unsupported ground exclusion, thickness/strength and cure holds, washout and the engineer's final re-entry release. An unresolved quality hold cannot pass at shift change.
STOP, WITHDRAW OR HOLD FOR MINE RELEASE
The source orders STOP / WITHDRAW / NOTIFY MINE CONTROL for support design deviations, abnormal ground movement or failed strength/lining holds. Restart only after release by the mine-authorised official.
ORIGINAL WORD SINGLE OR THE COMPLETE COLLECTIONS
The original editable AU-TT03 Word file is R12.95 and is already included in the 371-form Underground Mining Checklist Package and full 628-page Contractor H&S File. Existing parent-product owners need not buy twice. Live mine permits and rock-engineering approvals still govern use.
