WHAT THIS AIRPORT NDT ORIGINAL CONTROLS
AV-H01 is the authentic INSPECTION AND HOLD/RELEASE RECORD for airport inspection, NDT, testing and commissioning. A pressure, electrical or radiographic test may produce a satisfactory instrument result but still leave an unsafe bypass or live-service condition. This original uses SIX explicit HOLD/RELEASE points, independent witness evidence and actual operational-owner sign-off before an affected airport asset can be returned to use.
WHEN AND WHO COMPLETES IT
Use at the inspection-and-test-plan HOLD point, before temporarily excluded test areas are reopened or modified control systems go back into service. Enter AIRPORT/AREA, CONTRACTOR, WORK ORDER and AREA/SHIFT. Have the designated person review current test methods, calibration, exclusions, independent results and the evidence that settings or safety protections have been restored. Leave the hold active until the competent custodian documents actual acceptance.

The SIX real gates are (1) approved INSPECTION/ITP and SERVICE WINDOW; (2) COMPETENT TEST METHOD and CALIBRATION CHECK; (3) temporary EXCLUSION/ENERGY ISOLATION APPROVAL; (4) independent TEST/DEFECT RECORDS; (5) RETURN-TO-NORMAL FUNCTIONAL WITNESS; (6) OPERATIONAL OWNER SIGNOFF. Do not describe eight gates or imply this contractor record is a specialist radiography permit or actual calibrated NDT test report.
SOURCE-SPECIFIC EVIDENCE AND RELEASE BOUNDARY
Each of the six source rows contains HOLD/RELEASE POINT, EVIDENCE REF., ACCEPTED BY and DATE/SIGN. The closing lines require HOLD/NCR/DEVIATION REFERENCE and INDEPENDENT CHECK/SITE RELEASE REFERENCE. No signature may be invented: record the responsible airport reviewer and genuine evidence. Work remains on hold until the designated authorised person records the applicable release.
