WHAT THIS AIRPORT NDT ORIGINAL CONTROLS
AV-C00 is the source-original EIGHT-CONTROL CONTRACT AND ANNEXURE ACTIVATION form for airport inspection, NDT, testing and commissioning. An inspection annexure should be activated only where the real contract allows that specialist scope, the asset owner knows the proposed testing sequence and the contractor can demonstrate competent personnel and approved hazard controls. The final start is jointly accepted by the airport-area/system owner and contractor supervisor; it is never a general permission to conduct radiography.
WHEN AND WHO COMPLETES IT
Use on first activation of AV inspection/testing trade works under a named airport work order. Complete AIRPORT/AREA, CONTRACTOR, WORK ORDER and AREA/SHIFT. Establish the actual asset and test schedule, calibrated equipment, affected airlines/operations, permitted pressure/electrical/radiation work and the separate radiography controls when required. Revisit the approved workfront and permit boundaries when changes occur.

A missing equipment certificate, accepted test programme or radiographic permission is a restriction that must be resolved by competent people.
