335 / AU-R03 — DUST HYGIENE WORK RESULTS, MEASUREMENTS & EVIDENCE

AU-R03 is AVC's original one-page editable Word measured work-results register for surface-mine dust and occupational hygiene checks. It is designed for actual mine-controlled evidence IDs, sample results and assessed exposure values—not an unmeasured tick sheet.

WHAT CAN GO WRONG WITHOUT THIS EVIDENCE

Visibly reduced dust at a crusher does not prove safe respirable silica levels. Incorrect flow rates, missing sample chain-of-custody or a sudden wind change can undermine a mine hygiene conclusion. This record links test values to calibrated instruments and corrective assessment.

SEVEN ACTUAL PRINTED EVIDENCE ROWS

1. Mine hygiene officer specifies dust/silica sampling scheme, locations and methods. 2. Road watering, water carts and mist suppression compatible with road grip. 3. Sample pumps, flow meters, instruments and lab traceability calibrated. 4. Identify respirable silica, dust, diesel fume and noise exposure routes.

5. Assess wind direction, enclosures and process suppression. 6. Document monitoring results, exceedances and corrective actions against mine thresholds. 7. Report any failed crushing/drilling suppression for immediate reassessment.

USING THE SOURCE WORD FIELDS AND MINE RELEASE

Complete MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Each MINING-SPECIFIC ITEM / EVIDENCE TO TRACK has DOCUMENT / RESULT / MEASURED VALUE and DATE / AUTHORITY / STATUS. The original explicitly calls for sample chain of custody, calibration evidence, monitoring reports and hygiene review. Enter original document IDs and actual measured results against approved mine thresholds. Do not treat this worksheet as a laboratory certificate or a substitute for a competent hygiene assessment.

Use OUTSTANDING DEFECT / NCR / REINSPECTION / RETENTION FILE REF for open controls. CONTRACTOR SUPERVISOR / SIGNATURE and MINE RELEASE AUTHORITY / SIGNATURE are separate, with DATE / RELEASE REF. A contractor may not self-release an affected mine zone.

BUY THE SINGLE OR THE COMPLETE MINING SYSTEM

The identical original DOCX is ALREADY INCLUDED in both the 386-document Surface Mining Checklist Package and the full Contractor H&S File, which also provides management controls and trade annexures. Buyers of either collection already receive it. This Single is purchaser-licensed for internal project use under host-mine procedures.
