334 / AU-R02 — DUST HYGIENE DEFECTS, NCR & VERIFIED CORRECTIVE ACTION
AU-R02 is the original editable Word mine dust-suppression defect and NCR record. Its seven source checks document the evidence required after an instrument fault, exceedance, failed crusher spray or other mine hygiene nonconformance.
WHAT CAN GO WRONG WITHOUT THIS EVIDENCE
A failed suppression system cannot be closed simply because a pump runs again. A mine may require exposure reassessment, valid instrument calibration, recorded threshold results and witnessed remedial testing before the affected zone is released.
SEVEN ACTUAL PRINTED EVIDENCE ROWS
1. Mine hygiene/environment authority witnesses restoration and accepts measurements. 2. Report failed crushing/drilling dust suppression for immediate reassessment. 3. Monitor exceedances and corrective actions against mine thresholds. 4. Assess wind, enclosures and process suppression in exposure.
5. Identify silica, dust, diesel fumes and noise exposure for crews. 6. Sampling pumps, flow meters, laboratory evidence calibrated and traceable. 7. Verify road watering and mist suppression do not impair road grip.
USING THE SOURCE WORD FIELDS AND MINE RELEASE
Complete MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Each MINING-SPECIFIC ITEM / EVIDENCE TO TRACK has DOCUMENT / RESULT / MEASURED VALUE and DATE / AUTHORITY / STATUS. Use the printed OUTSTANDING DEFECT / NCR / REINSPECTION / RETENTION FILE REF to connect the independent NCR or corrective workpack. The original does not print an issue-owner/due-date matrix. Attach the failed reading, rectification, measured reinspection and hygiene authority evidence rather than asserting compliance.
Use OUTSTANDING DEFECT / NCR / REINSPECTION / RETENTION FILE REF for open controls. CONTRACTOR SUPERVISOR / SIGNATURE and MINE RELEASE AUTHORITY / SIGNATURE are separate, with DATE / RELEASE REF. A contractor may not self-release an affected mine zone.
BUY THE SINGLE OR THE COMPLETE MINING SYSTEM
The identical original DOCX is ALREADY INCLUDED in both the 386-document Surface Mining Checklist Package and the full Contractor H&S File, which also provides management controls and trade annexures. Buyers of either collection already receive it. This Single is purchaser-licensed for internal project use under host-mine procedures.
