333 / AU-R01 — DUST HYGIENE COMPETENCE, PERMITS & PLANT IDENTIFICATION
AU-R01 is AVC's original one-page editable Word workpack record for mine dust suppression and hygiene-monitoring competence, permits and equipment identification. It ties a specific mine area, work order and supervising contractor to real monitoring and safety-control evidence.
WHAT CAN GO WRONG WITHOUT THIS EVIDENCE
Qualified personnel still need an authorised sampling scheme, calibrated and identifiable monitors, and a mine-approved work area. A missing calibration certificate or failure to identify a silica exposure route may make a workpack unsuitable for release.
SEVEN ACTUAL PRINTED EVIDENCE ROWS
1. Mine hygiene officer specifies dust/silica sampling plan and locations. 2. Water-cart and mist suppression compatible with haul-road grip. 3. Sampling pumps, flow meters, instruments and laboratories calibrated. 4. Crew exposure to silica, dust, diesel fume and noise identified.
5. Wind, enclosures and process suppression included in exposure assessment. 6. Monitoring results, exceedances and corrective actions tracked against thresholds. 7. Any impaired crushing/drilling suppression immediately reported for reassessment.
USING THE SOURCE WORD FIELDS AND MINE RELEASE
Complete MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Each MINING-SPECIFIC ITEM / EVIDENCE TO TRACK has DOCUMENT / RESULT / MEASURED VALUE and DATE / AUTHORITY / STATUS. The printed evidence table uses DOCUMENT / RESULT / MEASURED VALUE and DATE / AUTHORITY / STATUS. Reference actual sampler identity, laboratory calibration, hygiene-officer instruction, training and mine permissions in the supporting evidence. It does not have printed personal licence or serial-number columns.
Use OUTSTANDING DEFECT / NCR / REINSPECTION / RETENTION FILE REF for open controls. CONTRACTOR SUPERVISOR / SIGNATURE and MINE RELEASE AUTHORITY / SIGNATURE are separate, with DATE / RELEASE REF. A contractor may not self-release an affected mine zone.
BUY THE SINGLE OR THE COMPLETE MINING SYSTEM
The identical original DOCX is ALREADY INCLUDED in both the 386-document Surface Mining Checklist Package and the full Contractor H&S File, which also provides management controls and trade annexures. Buyers of either collection already receive it. This Single is purchaser-licensed for internal project use under host-mine procedures.
