AU-H02 — SHOTCRETE INDEPENDENT WITNESS HOLD AND RELEASE
AU-H02 is the source-original five-gate independent-witness hold and release form for underground sprayed concrete, grout and rehabilitation. The contractor cannot grant a mine re-entry permission because a spray layer appears complete: separate proof and witnesses are required for all applicable hold gates.
WHERE AND WHEN THIS SOURCE REGISTER IS USED
Use it at mine-controlled quality, support, ventilation, ground access or dewatering hold points before another contractor or mine crew enters the lined zone. Enter MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Operator-owned release is recorded by reference only.

HOW TO COMPLETE THE GENUINE TABLE
The real witness table has HOLD TRIGGER / EVIDENCE GATE, REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. Capture actual test records, safe access proof, verification results and the responsible named witness for each of five source printed conditions; an absent witness is not an accepted gate.
DEFECTS, SIGN-OFF AND OPERATOR RE-ENTRY
All applicable hold items must PASS, proof must trace to the approved mine work order, and defects must be corrected and witnessed. Contractor supervisor and host mine authority signatures and the Date / release reference document only the actual mine-official release.
