AU-H02 is the original one-page editable Word independent witness and handback hold-point register for mine dust suppression and occupational hygiene works. It contains FIVE evidence gates, each requiring a result and witness, before a mine official releases the affected work area.
WHAT CAN GO WRONG WITHOUT THIS EVIDENCE
A successful visual inspection of a repaired crusher spray does not prove sampling validity, acceptable silica exposure or a safe restart. Independent checking of calibrated instruments, wind and process conditions, threshold results and suppression faults is needed.
FIVE ORIGINAL INDEPENDENT EVIDENCE GATES
1. Verify calibrated sampling pumps, flow meters, instruments and lab traceability. 2. Identify silica, dust, diesel fume and noise exposure pathways. 3. Account for wind, process enclosures and suppression effects.
4. Record monitoring results, exceedances and threshold corrective action. 5. Report suppression impairment during drilling/crushing for reassessment.
USING THE SOURCE WORD FIELDS AND MINE RELEASE
Complete MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Each printed hold gate has an evidence/verification and result/reference/witness field. Unlike the seven-row measured-results registers, AU-H02 prints FIVE HOLD TRIGGER / EVIDENCE GATE items with REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS columns. Its RELEASE rule requires all gates verified, defects resolved and a valid authorised mine work release.
RELEASE is conditional upon all gates and defects being closed. CONTRACTOR SUPERVISOR / SIGNATURE and MINE RELEASE AUTHORITY / SIGNATURE are separate, with DATE / RELEASE REF. A contractor may not self-release an affected mine zone.
