AU-F01 is the original five-gate approved-work-package and hold/release record for mine shotcrete, lining and grouting. It requires genuine work-order proof and named witnesses before the mine can accept this stage of rehabilitation.

Use it at a defined mine heading or sprayed-support workface before releasing a contractor's approved work package. Enter MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Link each gate to the current operator-owned work order and specific lining zone.
