331 / AU-F01 — DUST HYGIENE TECHNICAL HOLD AND WORK RELEASE
AU-F01 is the original one-page editable Word mine technical HOLD and release record for dust-suppression and occupational hygiene works. It deliberately has FIVE critical evidence gates, each requiring documented verification and a named witness before the mine releases work.
THE TECHNICAL EXPOSURE AND CONTROL PROBLEM
A dust monitor in place or a recently repaired spray does not demonstrate an acceptable exposure environment. Mine workers can still face respirable silica, unreliable sampling readings, diesel fumes and road grip hazards caused by excess watering. AU-F01 makes those gates explicit before a mine authority handback.
FIVE PRINTED INDEPENDENT RELEASE GATES
1. Mine hygiene officer establishes dust/silica sampling scheme, locations and methods. 2. Water carts, road watering and mist suppression compatible with safe road grip. 3. Sampling pumps, flow meters, instruments and laboratory traceability correctly calibrated.
4. Respirable silica, dust, diesel fumes and noise exposure routes identified for crew. 5. Wind direction, enclosures and process suppression considered in exposure assessment.
HOW THE ORIGINAL WORD FORM IS COMPLETED
Record the mine work area, work order, shift/permit and contractor supervisor on the source form. Each actual HOLD TRIGGER / EVIDENCE GATE has REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. Attach the mine hygiene officer's sampling instruction, current calibrations, exposure and wind review, and proof of vehicle-road safety. The form does not invent dust readings or replace a measured-results report.
RELEASE is allowed only when ALL five gates have been verified, defects resolved and an authorised mine official records a valid work release. The contractor supervisor and mine release authority sign separately with DATE / RELEASE REF; a supervisor's signature is not mine permission.
ONE SINGLE OR THE COMPLETE MINE COLLECTION?
This original purchaser-licensed editable Word form is INCLUDED in both the 386-document Surface Mining Checklist Package and the complete Surface Mining Contractor H&S File with additional core controls and specialist annexures. Existing buyers of either full set already have it. Follow your mine's current hygiene programme, exposure limits and authorisations.
