AU-C02 — SHOTCRETE SHIFT AND AREA READINESS CHECKLIST
AU-C02 is the original seven-row shift and area readiness checklist for underground sprayed concrete, resin/grout and rehabilitation. It verifies that each incoming crew has valid material, high-pressure spray, ventilation and ground support controls before resuming.
WHEN THIS MINE FORM APPLIES
Use it for each new mine lining shift or changed workface, with MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. Compare the live spraying system and current batch ticket to the approved mine instruction.
SEVEN PRINTED WORKFACE CONTROLS
1. Material mix, batch, expiry, set time and chemical hazards recorded
2. High-pressure hose, rebound and splash exposures controlled
3. Ventilation and dust/mist capture adequate for spray and curing operations
4. No workers exposed to unsupported ground during substrate preparation
5. Substrate tests, thickness checks, strength verification and cure holds defined
6. Runoff and washout protected against mine drainage and contamination
7. Mine engineer signs thickness, strength and zone re-entry release
HOW TO COMPLETE THE EXACT SOURCE FIELDS
The actual source table has WORKFACE VERIFICATION / REQUIRED CONTROL, Y, N, N/A and EVIDENCE / DEFECT / ACTION. Tick genuine conditions for all seven rows, explain every NO or N/A and support the answer with the current approved mine work order and operator limits.
WHY THIS SOURCE RECORD MATTERS
This form begins with mix batch/expiry, then hose rebound, dust/mist, unsupported ground, substrate/cure tests, runoff and finally signed engineer re-entry. A mixer ticket alone does not clear failed lining strength or grant mine entry.
STOP, REVIEW AND MINE AUTHORITY
The source HOLD instruction requires stopping for missing mine clearance, isolation, emergency system, ground support or critical proof. Record review/close-out evidence and contractor and host mine signatures where required; approval remains mine-owned.
EDITABLE WORD OR COMPLETE UNDERGROUND COLLECTIONS
The source-identical AU-C02 Word original is R12.95. This form is already included in the 371-document Underground Mining Checklist Package and complete 628-page Underground Mining Contractor H&S File, so owners of either parent do not need to buy it twice. Current mine COPs, valid permits and competent appointments govern use.
