326 / AT-TT04 — FUEL/LUBRICANT/REFUELLING SHIFT TRANSFER & CONTROLLED RESTART
AT-TT04 is AVC's original one-page editable Word shift-transfer and controlled-restart toolbox talk for mine fuel farms, lubricant servicing and refuelling maintenance. The six printed checks are intentionally led by mine fuel/fire authority approval and proven equipment safety, not merely crew sign-off.
WHY THIS MINE FUEL CONTROL MATTERS
A transfer line can look ready while a bund remains unaccepted, an overfill device is untested, a hose lacks bonding or a tank atmosphere is unknown. Incoming crews require positive handover of restoration evidence, fire coverage, isolation and gas-testing permissions before starting transfer or maintenance.
THE SIX ACTUAL PRINTED TOOLBOX TALK POINTS
1. Mine fuel/fire authority signs restored bund integrity and resumed transfer. 2. Meter calibration, overfill cutout, emergency stop and leak-test evidence recorded. 3. Flammable-liquid sampling or tank entry only with mine-approved permit and gas testing.
4. Spill booms, bunds, contaminated-soil arrangements and fire coverage ready. 5. Static bonding, earthing, lightning protection and transfer hoses inspected. 6. Fuel tanks, pump pressure, electrical, hydraulics and lines safely isolated and drained.
THE ORIGINAL 16-SIGNATURE ATTENDANCE AND STOP RULE
Enter MINE / AREA / DATE / SHIFT and PERMIT / PRESENTER, discuss crew duties and changed conditions, then use all 16 NAME / EMPLOYEE OR ACCESS NO. and SIGNATURE rows. AT-TT04 places the mine fuel/fire authority's restoration sign-off FIRST, followed by the metering/overfill/leak evidence and permit-controlled tank sampling. That ordering distinguishes the controlled-restart talk from the pre-start or failure-mode versions. Discuss each required document and who has release authority; a toolbox briefing never grants transfer permission.
The printed STOP / WITHDRAW / NOTIFY MINE CONTROL rule applies to a leak, uncontrolled ignition risk, unknown tank atmosphere or failed overfill cutout. Restart ONLY after mine-authorised official release; this talk cannot approve tank entry or transfer.
THIS SINGLE OR THE FULL MINE COLLECTION?
The exact original purchaser-licensed editable Word file is ALREADY INCLUDED in the 386-document Surface Mining Checklist Package and in the full Surface Mining Contractor H&S File with core controls and trade annexures. Existing larger-set buyers already receive this form. Follow the mine's current fuel/fire, hazardous-area and work authorisation rules.
