302 / AS-H02 — TAILINGS & STOCKPILE INDEPENDENT INSPECTION, WITNESS AND HANDBACK
AS-H02 is the original editable Word five-gate independent inspection and witnessed handback hold form for waste-rock stockpile and tailings perimeter works. It keeps contractor verification separate from the mine's engineering/TSF decision to return a plant or zone to service.
THE REAL GEOTECHNICAL RISK
A small excavation can damage monitoring instrumentation or weaken a tailings drain, while an apparently dry slope still needs attention to stormwater triggers. The five independent gates verify operating limits, surveyed buried systems, instrument protection and downstream emergency readiness before release.
FIVE INDEPENDENT EVIDENCE GATES
1. Tipping, trenching and pipework confined to engineer-approved limits. 2. Weather and stormwater-diversion triggers monitored for erosion or overtopping. 3. Buried drain, decant/underdrain and monitoring positions surveyed before digging.
4. Instruments, piezometers and survey markers protected and tracked. 5. Emergency withdrawal routes and downstream exposure communicated to contractors.

The printed FIVE HOLD TRIGGER / EVIDENCE GATE rows have REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. RELEASE requires all five verified, defects resolved and a valid work release by an authorised mine official; no contractor signature can substitute for that approval.
