WHAT THIS SOURCE-ORIGINAL LIFTING FORM CONTROLS
AS-H01 is the original SIX-GATE INSPECTION AND HOLD/RELEASE RECORD for lifting, rigging, cranes and MEWP work inside a controlled airport. The height of a raised crane can conflict with protected aerodrome obstacle surfaces even if the mechanical lift was technically correct. The work must also show approved lift plans, safe ground support, controlled lifting witnesses and the removal of residual FOD before requesting the airport operator's own release.
WHEN AND HOW TO COMPLETE IT
Open at a designated airport crane or mobile platform STOP/HOLD point. Enter AIRPORT/AREA, CONTRACTOR, WORK ORDER and AREA/SHIFT. Verify flight-operations height clearance, actual certified lift plan and load, pad or surface conditions, exclusion controls, competent witness, and whether all crane/MEWP equipment has been removed. Do not close a hold based solely on the contractor's completion or an unverified obstacle-clearance assumption.

Record the actual airport custodian, relevant evidence and any open defects.
