297 / AS-00 — TAILINGS & STOCKPILE WORK-ORDER ACTIVATION AND MINE ACCEPTANCE

AS-00 is the original editable one-page Word work-order activation and mine acceptance form for authorised waste-rock dump, stockpile and tailings-perimeter jobs. Six source control questions link geotechnical permission to documented evidence and an accepting owner before contractor mobilisation.

WHY THIS ENGINEERING CONTROL MATTERS

A construction work order does not itself approve a TSF boundary, crest access or a trench across a buried drain. The responsible engineer must define the allowed working envelope and verify ground, water and monitoring interfaces before the owner accepts the scope. This form records those six activation decisions.

SIX PRINTED ACTIVATION QUESTIONS

1. Responsible tailings/dump engineer defines contractor exclusion and work envelope. 2. Crest and foundation stability, seepage, settlement and access by current plan. 3. Tipping, trenching and pipework only under engineering limits.

4. Weather and stormwater diversion monitored for erosion or overtopping. 5. Buried drains, decant/underdrain and instrument locations surveyed before digging. 6. Instruments, piezometers and survey markers protected and tracked.

ACTUAL EVIDENCE FIELDS, HOLD AND MINE AUTHORITY

Identify MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. The original is a THREE-COLUMN form: ACTIVATION / CONTROL QUESTION, DOCUMENT / PERMIT / EVIDENCE REFERENCE and OWNER / ACCEPTANCE. It is NOT a seven-row Y/N checklist or a structural design inspection. Record the mine-approved work zone, evidence IDs and actual acceptance owner for each question.

The original REVIEW / CLOSE-OUT has separate contractor supervisor and mine release authority signatures and DATE / RELEASE REF. The contractor cannot self-authorise its work zone.
