AR-H02 — RAIL INDEPENDENT WITNESS HOLD & RELEASE RECORD
AR-H02 is the original five-gate independent witness hold and release form for track and locomotive support work. It separates a contractor's technical completion from mine-controlled acceptance of vehicle, shunting, workface and track-testing safety.
WHERE AND WHEN THIS SOURCE IS USED
Use it during inspection or transfer of an approved underground rail workpack, particularly before the mine releases a protected section. Enter MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR for the actual work area.
FIVE GENUINE RAIL WITNESS HOLD GATES
1. Rail-bound vehicle brakes, couplings and passenger rules checked
2. Shunting plan, lookout and radio protocols agreed with traffic controller

HOW TO FILL THE ORIGINAL WORD TABLE
