WHY ORIGINAL HAUL-ROAD EVIDENCE IS IMPORTANT
After a diversion or traffic-system change, a contractor may assume a reopened road is safe while spotter separation, warning signage or truck sight lines remain unsuitable. This form requires evidence/witnesses for the physical route and traffic-system acceptance, not just the works supervisor's confidence.
