AR-F01 is AVC's original editable five-gate Word mine technical HOLD and work-release document for live haul roads, fleet traffic and dump-point infrastructure. The FIRST verification gate belongs to the mine roads owner, who must approve traffic routing, intersections and active diversions before contractor work or reinstatement can be accepted.
THE MINE-SPECIFIC RISK AND SAFETY DECISION
A road may appear open after maintenance but the haul truck turning envelope, berm bearing, collision sight line or radio separation from light vehicles can still be inadequate. Weather and dust limits may differ from the previous shift. The original five-gate review prevents a contractor from using an ordinary site-inspection signature as permission to return a route to mine haulage.
FIVE ORIGINAL TECHNICAL HOLD GATES
1. Mine roads owner approves the traffic plan, intersections and active route diversions. 2. Check dump-edge berm, truck approach angle and tip-head bearing before access. 3. Spotter presence, radio protocol and exclusion distance for light vehicles controlled.
4. Road width, grade, curvature, sight lines and speed class suit the actual truck envelope. 5. Rain, dust, ice, fog and visibility-driven restrictions communicated to each shift.
HOW TO COMPLETE THE REAL WORD DOCUMENT
Each actual HOLD TRIGGER / EVIDENCE GATE requires REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. The form records five checks—not seven—and explicitly includes the dump-point approach and the roads owner's route plan. Attach the authorised traffic plan, dump-berm assessment, radio/spotter evidence and current weather restriction. Do not invent a berm height or speed limit.
