TECHNICAL OUTAGE PURPOSE
Source-original AT-H02 editable Word for Testing, Inspection, NDT and Commissioning at a power station or industrial outage. Its purpose is to control actual plant scope, work package, permit, technical evidence and witness decisions. WHEN TO COMPLETE THE ORIGINAL
Use on the actual CONTRACT / UNIT, WORK PACKAGE / ASSET, DATE / SHIFT and PERMIT / REVISION. Confirm the correct asset tag, operating area custodian, competent inspector, approved test plan, exclusion zone, simultaneous activities and current authorisation. Record objective measurements and deviations, not assumptions. If station conditions change, stop and obtain fresh facility instruction.

Original printed 4 technical witness HOLD gates: 1. Approval of hazardous test boundary and safety case; 2. Witness of pressure or high-energy functional test; 3. Acceptance of failed/repaired item before repeat test; 4. Final energisation/pressurisation/commissioning handover.
REQUIRED EVIDENCE OR ATTENDANCE
Record four HOLD/ACCEPT gate decisions with ITP, actual required evidence and a competent named witness; preserve the independent station release reference and unresolved NCRs.
STATION AUTHORITY AND RELEASE
Only the facility work-area owner controls current permits, high-energy isolation, switching, test approvals and return to service. Unverified readings, temporary connections, nonconforming material and missing witnesses remain HOLD. Preserve actual approval and handover documents, and never regard this contractor aid as an operator permit.
SHIFT AND CLOSEOUT CONTROL
Before handback, account for all loose tools, temporary connections, guards, test equipment, defects and permits. Keep current shift, contractor supervisor, engineering disposition, accepted test evidence and release references with the actual work package. Retain every open action for formal review.
